1. Who is the underwriter of this plan?

Delta Dental Insurance Company (Delta Dental).

2. Who is the administrator of this plan?

Delta Dental Insurance Company and Morgan-White Administrators, Inc. Morgan-White will market the plan to brokers, contract with brokers who are appointed with DDIC, issue evidence of coverage booklets and I.D. cards, collect premiums, pay commissions, and handle customer service related to billing and marketing for the Association.

3. When is the last day an application can be received in Morgan-White office and be effective for the first of the next month?

If the application is complete and received with payment by the 20th of the month, coverage will be effective the first of the next month.

Billing

1. For bank draft billing, what day does the bank draft take place?

Between the 18-23rd of each month for the upcoming month's premium..

2. Can this plan be list billed?

Yes, but it requires a minimum of 2 enrolled members to make a list billing.

3. If a member is on a list bill and leaves that group (billing location), does he or she have an option of taking along the coverage?

Yes, the member can be set up on a monthly bank draft or any billing currently available.

4. Is there a rate guarantee on this product?

Yes, 6 months.

5. Is it possible to have a two year rate guarantee?

No.

1. Are missing teeth covered?

No.

Yes.

They will be mailed to the member's home or emailed to the member’s email address.

Benefits Association, Inc.

1. How do we get more information about the other services the member gets when he or she joins the Benefits Association?

Log onto www.benefitsassociation.com and then "Who is BAI" to learn about the reasons for the Association and then "Advantages" to learn about the other benefits of joining the Association.

2. Can I join the Benefits Association without buying the dental insurance?

Yes. You enroll and pay the $12 per year membership.

3. Can I have the dental coverage without joining the Benefits Association?

No, the group policy is issued to the Association.
